1. MEDICAL RESPONSIBILITY & FIRST AID AUTHORIZATION

I understand that HYO Martial Arts, its staff members, and event personnel are not medical professionals.
I authorize HYO Martial Arts staff to administer basic first aid in the event of injury or illness during the birthday party.

If deemed necessary, I authorize HYO Martial Arts staff, volunteers, or emergency services (ambulance) to seek medical treatment and transport my child to any medical facility or hospital.

2. ACKNOWLEDGMENT AND ACCEPTANCE OF RISKS

I understand that participation in a birthday party event may include physical activities, games, and group play, which involve inherent risks.
These risks may include, but are not limited to, minor or serious injuries such as bruises, sprains, fractures, or other unforeseen injuries.

I acknowledge that these risks may result from:

• My child’s actions

• The actions of other participants

• Facility conditions

• Or the negligence of HYO Martial Arts or its representatives

I voluntarily accept all such risks and assume full responsibility for any loss, cost, or damage arising from my child’s participation in the birthday party.

3. RELEASE OF LIABILITY & INDEMNIFICATION

I hereby release, discharge, and agree not to sue HYO Martial Arts, any facility used for the event, their owners, directors, officers, instructors, staff members, volunteers, representatives, sponsors, and agents (collectively, the “Releasees”) from any and all claims, liabilities, demands, or damages arising out of or related to participation in the birthday party, whether caused in whole or in part by the negligence of the Releasees or otherwise.

If any claim or legal action is brought by me or on behalf of my child, I agree to indemnify and hold harmless all Releasees from any and all litigation expenses, attorney fees, liabilities, and damages.

4. PARTY RULES, CONDUCT, AND FACILITY POLICIES

I understand and agree that my child must follow all party rules and instructions provided by HYO Martial Arts staff during the event.

I acknowledge that:

• HYO Martial Arts is not responsible for lost, stolen, or damaged personal property.

• HYO Martial Arts reserves the right to remove any participant from the party for unsafe behavior, violence, fighting, or unlawful conduct, without refund.

5. MEDIA RELEASE

I grant permission to HYO Martial Arts to photograph and/or record video of my child during the birthday party and related activities.
I authorize the use of such photos or videos for promotional, marketing, social media, and website purposes.

(If you do not agree, written notice must be provided prior to the event.)

6. AGREEMENT AND ACKNOWLEDGMENT

I confirm that I have read and fully understand this Birthday Party Waiver and Release Form.
I voluntarily agree to all terms stated above and consent to my child’s participation in the HYO Martial Arts birthday party.
