PARTY ATTENDANCE FORM

PARTY WAIVER & CONSENT

Assumption of Risk, Parental Consent (if applicable), and Indemnity Agreement

1. ASSUMPTION OF RISK

 

I understand that my child will participate in physical activities during the birthday party, including but not limited to martial arts games, running activities, board breaking, climbing, and other recreational movement.

 

I acknowledge that participation in these activities involves inherent risks of injury, and I voluntarily assume all such risks on behalf of my child.

 

 

2. RELEASE OF LIABILITY

 

I hereby release, waive, and hold harmless HYO Martial Arts, its owners, instructors, staff, and volunteers from any and all claims, injuries, damages, or liabilities arising from my child’s participation in birthday party activities, except in cases of gross negligence.

 

 

3. MEDICAL EMERGENCY AUTHORIZATION

 

In the event of an injury or medical emergency, I authorize HYO Martial Arts staff to administer basic first aid and to seek emergency medical treatment if necessary, including transportation by emergency services if deemed necessary.

 

I understand and agree that I am financially responsible for any medical expenses incurred.

 

 

4. MEDICAL & SAFETY INFORMATION CONFIRMATION

 

I confirm that I have fully and accurately provided all relevant medical conditions, allergies, and safety-related information regarding my child in the Birthday Party Attendance Form submitted prior to this waiver.

 

 

5. PHOTO & VIDEO CONSENT

 

I understand that photos and/or videos may be taken during the birthday party for documentation, safety, and promotional purposes.

 

I grant HYO Martial Arts full permission to use such photos or videos without compensation.

 

 

6. PARENTAL ACKNOWLEDGMENT & AGREEMENT

 

I confirm that I am the parent or legal guardian of the child listed above.

 

I have carefully read and fully understand this waiver.

 

I acknowledge that completion of this waiver is required for participation, and I voluntarily agree to all terms and conditions stated above.

 

 

IMPORTANT NOTICE

 

This waiver must be completed prior to the birthday party.

 Children without a completed waiver will not be permitted to participate in any activities.
